Your Baby's Birth Certificate

Baby’s here — now it’s time to make it official! Soon after delivery, one of Beaufort Memorial's birth certificate clerks will visit you and collect the information needed for your baby's birth certificate.

The clerk will ask:

• For your baby's full name
• About you, the baby's mother
• About the baby's father
• If you would like to request a social security number for your baby

To ensure the certificate is filled out correctly, you’ll need to have:

• Current photo IDs
• Social security numbers for both the baby's mother and father

If you're not married and want the father's name on your baby's birth certificate, you will need to establish paternity. We can help you with that process right at the Birthing Center, free of charge.
