It is important to make your end-of-life preferences known to those who are close to you. Use these forms to document your wishes:
• Healthcare Power of Attorney (English)
• Healthcare Power of Attorney (Spanish)
• South Carolina Living Will (English)
• South Carolina Living Will (Spanish)
Once you’ve filled out these forms, tell your loved ones where the original is kept and give a copy to your doctor. In addition, we can scan them and keep them on file in case you are hospitalized. To take advantage of this free service, bring your completed forms, insurance card and a photo ID to the admitting department.
