Back-to-School Vaccine Checklist
Protect your child’s immune system this fall. Make sure he or she meets the South Carolina immunization requirements for the 2020‑2021 school year.
Hepatitis A
- Grade: 5K
- 2 doses — first dose at age 1 with second dose at least six months later
- Protects from: hepatitis A
Hepatitis B
- Grade: 5K-12
- 3 doses — first dose received at birth, second dose at 1-2 months and third dose at 6-18 months
- Protects from: hepatitis B
DTaP
- Grade: 5K-12
- 4 doses — at least one dose received on or after fourth birthday
- Protects from: diphtheria, tetanus and pertussis (whooping cough)
MMR
- Grade: 5K-12
- 2 doses — first dose at 12-15 months and second at ages 4-6; catch-up vaccines should be at least four weeks apart.
- Protects from: measles, mumps and rubella
Polio
- Grade: 5K-12
- 3 doses — one dose received on or after fourth birthday
- Protects from: polio
Tdap booster
- Grade: 7-12
- 1 dose — received on or after seventh birthday
- Protects from: tetanus, diphtheria and pertussis (whooping cough)
Varicella
- Grade: 5K-12
- 2 doses — first dose between 12 and 15 months old and second dose between 4 and 6 years; catch-up vaccines should be at least four weeks apart. Parents can also provide proof of illness.
- Protects from: chickenpox
