A free program is being offered to BlueCross© BlueShield© of South Carolina members who receive care through a Beaufort Memorial physician. The Patient-Centered Medical Home (PCMH) program allows us to help you better understand and improve your health conditions.

And best of all, you don’t even have to travel – everything is completed online or by phone.

How PCMH Works

As an official BlueCross© BlueShield© of South Carolina PCMH partner, Beaufort Memorial is working closely with patients to:

  • Coordinate routine care and screenings
  • Manage medications and chronic conditions
  • Provide convenient access to care 24/7

Our goal is to assist with your current health care needs and prevent future health problems. 
 

One great benefit of the program is the option to work with a care coordinator who can assist you by providing education and support and helping you reach your health goals.

One Couple's Success with PCMH

Jim and Jean Brennan

For Sun City residents Jean and Jim Brennan, the PCMH program has improved their quality of life and provides the accountability they need to stay on track to reach their health and wellness goals. 
As part of the PCMH program, Jim and Jean are working with a care coordinator to improve their overall health. They have 20-minute conversations once a week to review their goals and check on their progress.

“It’s been a great journey working with a care coordinator,” says Jean, 64. “Having the encouragement and knowing that I’m going to be talking to somebody on Friday morning keeps me looking for new (healthy) recipes, making sure that I’m working out and taking care of myself. I need that kind of accountability.” 
The Brennan couple, married 30 years, receives health coaching, education and, at times, even emotional support. 
“I’m not sure I would feel as good as I do today if it wasn’t for having my care coordinator there,” says Jim, 63.

Jean’s Improvements

  • Eats three balanced meals a day
  • Visits the farmers market to add more fresh fruits and vegetables into diet
  • Exercises regularly (lost 30 pounds!)
  • Improved self-esteem
